If you are commercially insured and have pharmacy benefit coverage for the Medtronic continuous glucose monitors (CGM), you may be eligible. This program applies to:
- Instinct sensor, made by Abbott
- Guardian™ 4 sensor
- Simplera Sync™ sensor (for use with the MiniMed™ 780G system)
- Simplera™ CGM (for use with the InPen™ smart insulin pen system)
For the Guardian™ 4 transmitter, pay as little as $99 per transmitter! Applicable one time per year.
How it works
If you have insurance coverage for the Medtronic CGM through your pharmacy benefit, and meet other eligiblity terms, the copay assistance will be automatically applied at the pharmacy.

Who is eligible for the CGM Pharmacy Copay Program?
You may be eligible for this program if you meet the following requirements:
- You have commercial health insurance.
- You have a valid prescription for your Medtronic CGM from your healthcare professional.
- You have coverage for CGMs under your health plan’s pharmacy benefit.
What if I’m not eligible for this program?
Eligibility is determined based on insurance coverage. You can call us at 800-646-4633 (Monday - Friday, 8 a.m. - 6 p.m. CT) to verify both your medical benefit for supplies and your pharmacy coverage to determine your lowest out-of-pocket cost. If you do not have any insurance coverage for the prescribed CGM, you may be eligible for other financial support programs from Medtronic.
How does the copay program work?
This copay support program will be automatically applied to your out-of-pocket balance due when you obtain the CGM through the Medtronic Pharmacy or other pharmacy within the vendor network.
Will the copay support be applied to my coinsurance or deductible as well?
This copay support would be applied to the amount you owe when the pharmacy claim is adjudicated.
Which products are included in this program?
This program is valid for: the Instinct sensor, Guardian™ 4 sensor and transmitter, Simplera Sync™ sensor, and Simplera™ CGM.
CGM Pharmacy Copay Program Terms & Conditions
- Valid prescription from provider required.
- Eligible patients: Commercially insured patients only; with coverage through their pharmacy benefit.
- Excluded participants include patients enrolled in any federal, state, or government-funded healthcare program such as Medicare, Medicare Advantage, Medicare Part D, Medicaid, Medigap, Veterans Affairs (VA), the Department of Defense (DoD) or TRICARE® that provides coverage for G4S, or where prohibited by law (including MA and MI).
- If at any time a patient becomes enrolled under any such federal, state, or government-funded healthcare program, he/she will no longer be able to use this program and must call (858) 522-0252 to stop participation.
- For the Guardian™ 4 sensor and transmitter: patients can receive up to a total of $500 maximum annual benefit for their annual transmitter. Patients can receive up to a total of $720 annual benefit for their sensors, with a monthly maximum benefit of $180 per month.
- For the Instinct sensor, Simplera™ CGM, and Simplera Sync™ sensors: Patients can receive up to a total of $2000 annual benefit for their sensors, and a monthly maximum benefit of $380/month.
- Program valid for eligible patients filling a prescription after December 13, 2024. There is no retroactive application of this program.
- Customer agrees not to submit any claims for rebated products to any Government Plan and will not request their pharmacy or DME supplier to submit any such claim to any Government Plan.
- Restrictions may apply. If the Medtronic CGM is not covered on your plans formulary, this program will not apply.
- Offer and terms and conditions are subject to change or discontinue without notice.
- This is not health insurance.

Important Safety Information: MiniMed™ 780G system with SmartGuard™ technology with Instinct sensor, Simplera Sync™ sensor, and Guardian™ 4 sensor
The MiniMed™ 780G system is intended for the continuous delivery of basal insulin at selectable rates and the administration of insulin boluses at selectable rates for the management of type 1 diabetes mellitus in persons 7 years of age and older using the Instinct, Simplera Sync™ or Guardian™ 4 sensor, and of type 2 diabetes mellitus in persons 18 years of age and older requiring insulin using the Simplera Sync™ or Guardian™ 4 sensor.
The MiniMed™ 780G System includes SmartGuard™ technology, which can be programmed to automatically adjust insulin delivery based on the continuous glucose monitoring (CGM) sensor glucose values and can suspend delivery of insulin when the sensor glucose (SG) value falls below or is predicted to fall below predefined threshold values. The system is intended for use with connected sensors, including the Simplera Sync™ and Guardian™ 4 sensors and integrated continuous glucose monitors, including the Instinct sensor, each of which has different wear-time, form factor, insertion site, and other distinguishing characteristics that relate to sensor performance. Consult the appropriate sensor user guide when using the system. Discuss treatment decisions with your HCP.
WARNING: Do not use the SmartGuard™ feature for people who require less than 8 units or more than 250 units of total daily insulin per day. A total daily dose of at least 8 units, but no more than 250 units, is required to operate in the SmartGuard™ feature.
WARNING: Do not use the MiniMed™ 780G system until appropriate training has been received from a healthcare professional. Training is essential to ensure the safe use of the MiniMed™ 780G system.
WARNING: Do not use SG values to make treatment decisions, including delivering a bolus, while the pump is in Manual Mode. When the SmartGuard™ feature is active and you are no longer in Manual Mode, the pump uses an SG value, when available, to calculate a bolus amount. However, if your symptoms do not match the SG value, use a blood glucose (BG) meter to confirm the SG value. Failure to confirm glucose levels when your symptoms do not match the SG value can result in the infusion of too much or too little insulin, which may cause hypoglycemia or hyperglycemia.
Pump therapy is not recommended for people whose vision or hearing does not allow for the recognition of pump signals, alerts, or alarms. The safety of the MiniMed™ 780G system has not been studied in pregnant women, persons with type 2 diabetes using the Instinct sensor, or in persons using other anti-hyperglycemic therapies that do not include insulin.
