When can I redeem the Lift Ticket to upgrade to a new pump?

During your warranty period, with more than 6 months left, you will have a one-time opportunity to upgrade to another insulin pump for $99.

How do I redeem the Lift Ticket and how much will it cost?

To redeem your Lift Ticket, please call us at 800-646-4633 and select option 3. Specialists are available from 9AM-6PM CST. The total out-of-pocket cost to purchase a pump using a Lift Ticket is $99. You will initially be charged $499 and will receive a $400 credit once we receive your old pump.

How often can I upgrade using the Lift Ticket program?

The Lift Ticket program is a one-time in-warranty upgrade. Pathway programs may be available if you have already used your Lift Ticket.

Will my pump warranty reset after upgrading using the Lift Ticket program?

No, warranty dates do not reset. The warranty date of an upgraded insulin pump will remain the same as the warranty date of the initial pump purchased.

What if I do not return my old insulin pump within 30 days?

The $99 price will be honored if you comply with program terms. If you do not return your old insulin pump within 30 days, you will not receive the $400 credit and you will be charged $3,200.

What happens if I am already on the latest MiniMed™ insulin pump system (purchased before the launch of this program)?

† Lift Ticket Program Terms & Conditions

  • $499 charge will be required upfront to use Lift Ticket. Return of old insulin pump is required. Upon receipt of old pump, $400 will be credited to the original form of payment. If old pump not returned, patient will be charged $3,200.
  • After you have requested to use your Lift Ticket, it could take up to 60 days to process your order. The Lift Ticket Program does not guarantee prioritization of your order.
  • Lift Ticket can only be redeemed once during your warranty period. A Lift Ticket cannot be redeemed if less than six (6) months remains on your pump warranty period.
  • The Lift Ticket Program price applies to (1) insulin pump. CGM products and consumables are not included.
  • Insurance coverage and approvals may impact patient ability to upgrade. Coverage for your pump is contingent upon your payer medical policy guidelines. Restrictions and medical necessity requirements apply.
  • Warranty dates do not reset. The warranty date of an upgraded insulin pump will remain the same as the warranty date of the initial pump purchased.
  • Patients enrolled in a rental program must complete their final payment and own their pump before utilizing the Lift Ticket program.
  • MiniMed™ pumps have certain labeling restrictions that may exclude certain populations, including age and type of diabetes. We recommend you consult your Healthcare Provider.
  • A valid prescription will be required to purchase a new pump. Other restrictions may apply.
  • Program terms and conditions are subject to change at any time and without notice.
  • Any purchase for a patient with Out of Warranty pump or new to pump therapy of a Medtronic insulin pump that shipped between 11/14/18 and 4/11/19 is eligible to receive a Lift Ticket.

Important Safety Information: MiniMed™ 670G System with SmartGuard™ technology

The Medtronic MiniMed™ 670G system is intended for continuous delivery of basal insulin (at user selectable rates) and administration of insulin boluses (in user selectable amounts) for the management of type 1 diabetes mellitus in persons, seven years of age and older, requiring insulin as well as for the continuous monitoring and trending of glucose levels in the fluid under the skin. The MiniMed™ 670G system includes SmartGuard™ technology, which can be programmed to automatically adjust delivery of basal insulin based on Continuous Glucose Monitor sensor glucose values and can suspend delivery of insulin when the sensor glucose value falls below or is predicted to fall below predefined threshold values. The system requires a prescription. The Guardian™ Sensor (3) glucose values are not intended to be used directly for making therapy adjustments, but rather to provide an indication of when a fingerstick may be required. A confirmatory finger stick test via the CONTOUR®NEXT LINK 2.4 blood glucose meter is required prior to making adjustments to diabetes therapy. All therapy adjustments should be based on measurements obtained using the CONTOUR®NEXT LINK 2.4 blood glucose meter and not on values provided by the Guardian™ Sensor (3). Always check the pump display to ensure the glucose result shown agrees with the glucose results shown on the CONTOUR®NEXT LINK 2.4 blood glucose meter. Do not calibrate your CGM device or calculate a bolus using a blood glucose meter result taken from an Alternative Site (palm) or from a control solution test. It is not recommended to calibrate your CGM device when sensor or blood glucose values are changing rapidly, e.g., following a meal or physical exercise. If a control solution test is out of range, please note that the result may be transmitted to your pump when in the “Always” send mode.

WARNING: Medtronic performed an evaluation of the MiniMed™ 670G system and determined that it may not be safe for use in children under the age of 7 because of the way that the system is designed and the daily insulin requirements. Therefore this device should not be used in anyone under the age of 7 years old. This device should also not be used in patients who require less than a total daily insulin dose of 8 units per day because the device requires a minimum of 8 units per day to operate safely.

Pump therapy is not recommended for people whose vision or hearing does not allow recognition of pump signals and alarms. Pump therapy is not recommended for people who are unwilling or unable to maintain contact with their healthcare professional. The safety of the MiniMed™ 670G system has not been studied in pregnant women.
