When you call, we will verify your eligibility. After you have been verified, you can place future orders for CGM supplies by using www.Diabetes.Shop or by calling us.

If you are covered for Medtronic CGM under your insurance policy but your claim was denied for any reason, then you will not be eligible for CGM Access Discount. If you believe that your coverage was denied by mistake, we recommend checking with your insurance provider to understand your benefits. If you are unsure if Medtronic CGM is covered under your insurance policy, call us to verify your eligibility.
With the CGM Access Discount, the cost is $180 per transmitter (12-month warranty) and $60 per 5-pack box of sensors (1-month supply) for all eligible customers.
Once we confirm your eligibility, the discount will be applied to your order immediately. You may pay your balance at that time when you make a purchase with a credit card or you will be invoiced for the balance due in 30 days from when you place your order. Depending on your order, you may also be eligible for payment plan options.
Products that are eligible for this discount are Guardian Link 3 transmitter, Guardian Connect transmitter, and Guardian Sensor 3.

If using our CGM with a pump, the following pumps are compatible with the above products: MiniMed 770G system, MiniMed 670G system, and MiniMed 630G system.

Eligibility requirements

Customers must not have insurance coverage for the prescribed Medtronic CGM or are uninsured (i.e. self-pay). Customers must either be using multiple daily injections or be using an insulin pump. Customers must have a valid prescription for CGM supplies from their healthcare professional. Customers must verify their eligibility for the discount over the phone prior to placing any discounted CGM supplies order. If at any time customers become enrolled in an insurance plan that covers Medtronic CGM, they will no longer be eligible for this discount and must call 1-800-646-4633 Ext. 21063 (Monday- Friday, 8 AM - 6 PM CT). Orders must occur on or after 9/4/2020 for integrated CGM and 11/11/2020 for standalone CGM supplies until further notice. For the Guardian™ 4 sensor and transmitter program eligibility is as of 5/15/2023.

If you are making a submission to a federal healthcare program, such as Medicare, Medicaid, or Tricare, you must fully and accurately report in submissions to federal healthcare programs all discounts provided by Medtronic. Additionally, upon request by the Secretary of the U.S. Department of Health and Human Services or state agency, you must make available information provided to you by Medtronic concerning the discount.

Additional Terms and Conditions:
Enrolled customers will receive a discount for 1 transmitter per year and for 5 sensors per month. Additional supplies will not be eligible for a discount. A maximum of three (3) boxes of sensors (90-day supply) may be purchased per order. The transmitter will have a 12-month warranty. The discount applies to the following products: GuardianTM 4 sensor, GuardianTM Link 3 transmitter, GuardianTM Connect transmitter, and GuardianTM Sensor 3. Infusion sets, reservoirs, and other insulin pump supplies are not eligible for this discount. Customers may purchase said supplies within the same order as discounted CGM supplies, however, at regular list prices subject to insurance coverage and copays. Terms and conditions are subject to change at any time.

Exclusions:
Customers who have insurance coverage, either Medical/DME or Pharmacy Benefits, for Medtronic CGM are excluded. Customers who have insurance coverage for Medtronic CGM but failed to meet medical criteria are excluded.

Important safety information:

Medtronic Diabetes insulin infusion pumps, continuous glucose monitoring systems and associated components are limited to sale by or on the order of a physician and should only be used under the direction of a healthcare professional familiar with the risks associated with the use of these systems. Successful operation of the insulin infusion pumps and/or continuous glucose monitoring systems requires adequate vision and hearing to recognize alerts and alarms.

Medtronic diabetes insulin infusion pumps:

Insulin pump therapy is not recommended for individuals who are unable or unwilling to perform a minimum of four blood glucose tests per day. Insulin pumps use rapid-acting insulin. If your insulin delivery is interrupted for any reason, you must be prepared to replace the missed insulin immediately.

Medtronic diabetes continuous glucose monitoring (CGM) systems:

The information provided by CGM systems is intended to supplement, not replace, blood glucose information obtained using a home glucose meter. A confirmatory finger stick is required prior to treatment. Insertion of a glucose sensor may cause bleeding or irritation at the insertion site. Consult a physician immediately if you experience significant pain or if you suspect that the site is infected. Guardian sensors:

The Guardian 4 sensor is intended for use with the MiniMed 780G system. The Guardian Sensor (3) is intended for use with the MiniMed 770G system, MiniMed 670G system, MiniMed 630G system and the Guardian Connect system to continuously monitor glucose levels in persons with diabetes. The sensor is intended for single use and requires a prescription. The Guardian Sensors are indicated for seven days of continuous use.
